Unless you're planning to go through a clavicle piercing under general anesthetic, then yes, you can expect the experience to be somewhat painful. A local anesthetic will most likely be put to use but, that only affects the surface area for the most part - there's a chance the tattoo artist may be licensed to use sedatives for such purposes.
