Think of it this way, you have a bacterial infection break out amongst a community. They supply the community with amoxicillin, and it works at first. However, just like everything else, mutations involved in replication within the bacteria can cause antibiotic resistance. So let's say a few of the bacteria are exposed to amoxicillin and resist it, is contagious, and spreads the point of a second outbreak. At which point, treatment with amoxicillin is no longer effective due to said mutation.
